Illness Form
Every year a number of students become ill because of exposure to other ill children. We have listed indicators to help you decide. If your child displays any of these symptoms, it is suggested that you keep your child at home.

Fever over 100 degrees. A child should be fever-free for 24 hours without medication before returning to school.

Green nasal discharge. A child’s nose should be completely clear for 24 hours.

Persistent cough

Diarrhea. A child should be completely free of diarrhea for 24 hours before returning.

Nausea, vomiting

Pink eye or eye discharge

A rash of unexplained origin

If your child has been prescribed an antibiotic, they must be on it for 24 hours before returning to school.

Please Note:

Children will be sent home at the teacher’s discretion if any of these symptoms are displayed. We understand that parents have to go to work, but our main concern is the best quality care for your child.

Charges for late Pick-up
Family of Christ’s goal is to care for the children in the best possible way. Children become anxious when it is time to go home and no one has come for them. It is not only unfair to the child, but to the teacher as well because he/she cannot perform the pre-planning duties required at the time. There are times you will see the teacher in conversation with a parent, as you pick your child up. Even though the teacher is still in the classroom, this does not mean that she is able to watch a child whose parent is late in picking him/her up. For these reasons, a stringent late pick-up fee is assessed.

If my child remains at the school past the scheduled program closing time, a late fee of $5.00 per minute/per child will be charged. Late fees must be paid the first business day immediately following the date accrued.

Information Updates: If any information on your child's emergency forms change during the year, please stop by the front desk to make the corrections. The State of Arizona has strict licensing procedures.(Required)

Arrival & Departure Procedure: Children must be signed in and out on the sheets located at the front desk. A first initial, last name and time of day are mandatory. No one under 18 years of age may sign out and take a child. A child may only be released to the parent or adult listed on the emergency forms. In a case where an adult is unknown, picture ID may be required.(Required)

Release Slips: If someone other than yourself or your spouse is picking up your child, and is not on your emergency forms, you will need to fill out a form at the front desk.(Required)

Financial Agreement for Full-Day Pre-School
Tuition may be paid, bi-weekly or monthly. If paying weekly or bi-weekly, the tuition is due on the Monday of the pay cycle being covered. If paying monthly, the tuition is due on the first of the month being covered.(Required)

If a monthly payment has not been received by the fifth of the month, your account will be considered delinquent and a $25 late fee will be assessed. If the 5th falls on a weekend, payments received on the following Monday will not be considered delinquent.(Required)

Tuition is based on a yearly fee. No discount is given for holidays, illnesses, vacations, or absences and there will be no make-up days(Required)

A $25.00 fee will be assessed for any returned check, plus a $6.00 service charge will be added. In the case of a second returned check, all fees and tuition will be payable by cash or money order.(Required)

As a courtesy, we can email you an invoice showing the amount due at the start of each billing cycle. This email will include a link for paying electronically. At this point ACH payments (cash transfers from a checking account) will be the only form of electronic payment accepted and there will be a 1% fee charged.(Required)
